A hard copy of the Sports Physical MUST BE UPLOADED into the registration form prior to taking it to the Nurse's Office in order to be cleared to participate. Sports Physicals should be completed with your Family Doctor. If your family doesn't have access to a Family Doctor, you can go to an Urgent Care or Care Station.

Athletic Qualifications

ALL students MUST be both ACADEMICALLY and PHYSICALLY cleared in order to participate.

 

Physical Examinations

Students registering for Athletics must have a current physical examination on file in the nurse’s office before the student is authorized to participate in practice/tryouts.  To ensure clearance for participation, physicals should be completed and turned in to the nurse no later than 3 school days prior to the first day of practice/tryouts.
 
Physicals are valid for 12 months from the date of the last exam.
 

If the athlete has a current physical on file in the nurse's office, you DO NOT need a new physical. Please submit the Health History Update Form instead.
